CareSource has been supporting independence through excellent care for more than 15 years. We are an affiliate of the Visiting Nurse Association of South Central Connecticut, a local non-profit, home health care agency with more than 100 years of experience serving Greater New Haven, Milford and the lower Naugatuck Valley.
At the time of admission, we will come to your home and develops a care plan with you.
CareSource provides homemaker, companion, and live-in aide services for adults.
CareSource offers hourly, full shift and around-the-clock services in the hospital, home, and assisted living settings.
CareSource works with many insurance carriers.
CareSource is not licensed to bill Medicare or Medicaid. CareSource does participate with the Agency on Aging of South Central Connecticut to provide homemaking services primarily through the Connecticut Home Care Program for Elders Program.
CareSource participates with the veterans benefit program through the VA (Veterans Affairs)
CareSource provides you with an itemized bill for you to submit to your carrier.
CareSource has worked with various insurance companies to meet their provider criteria. Most long-term policies have a qualifying period, or a deductible and might pay a percentage of the cost for private duty services. We can assist you in determining your coverage parameters and work with your insurer to complete applicable paperwork.
Live-in service is 24 hours a day/7 days a week, and may be provided on an ongoing basis, or for standardized time periods (e.g. Monday-Friday) to allow family caregivers to work outside the home and assume weekend care. Live-in care can also be arranged as a respite service to relieve family caregivers once a month or for family vacations.
The live-in aide is not allowed to leave the home during this time without consent from CareSource so that the appropriate coverage can be arranged. The live-in aide sleep time is according to the client's schedule, and the live-in aide is expected to respond to the client's needs. The client will provide a place for the aide to sleep and eat meals.
The client is also expected to provide food for meals.
A rate schedule is available on request and is based on the type of service and hourly, shift or live-in rates.
All CareSource employees are bonded and insured.
