 What Is a Geriatric Care Manager?
A Geriatric Care Manager (GCM) is a health and
human services professional who provides guidance and assistance
to elders and their families as they navigate through the healthcare
system. They have experience in long-term care fields
such as nursing, social work, gerontology or psychology. A
GCM has a specialized focus on issues relating to aging and
elder care and disabled adults.
Geriatric Care management assistance is recommended
to any elder who has:

Multiple falls, unplanned and frequent
ER visits

Changes in memory, behavior or personality

Financial problems - including unpaid or over due
bills

Poor personal health care habits

Improper medication management
-
Loss of bowel and bladder function
Any senior who experiences any or all of the
above instances needs the support and guidance of a Geriatric

Payment
CareSource accepts payment directly
from clients, family members and conservators. All major
credit cards are honored. We do accept direct payment
from insurance carriers and will work closely with third party/private
insurance companies to maximize coverage and service.
